Guide

1. Click the "Create" button and select "Page" to start building your form in Confluence.

2. Type "/table" anywhere on the page, then select it from the menu to create a structured form layout.

3. Add form field labels in the left column and leave the right column blank for responses.

4. Click "Publish" to let users submit responses.
