1. Navigate to the 'Content' menu in your Joomla admin panel.

2. Click on 'Site Modules' from the dropdown menu.

3. Hit the '+ New' button to create a fresh module.

4. Select the 'Custom' module type.

5. Click on 'Toggle Editor' to access HTML mode for your form.

6. Click here to paste or type your HTML form elements.

7. Navigate to the 'Menu Assignment' tab to control where your form appears.

8. Select your display preference by clicking the drop-down arrow.

9. Go back to the 'Module' tab to continue configuring your form.

10. Click 'Toggle Editor' again to switch to visual mode and see how your form looks.

11. Review your form fields and make any needed adjustments.

12. Click in the title field to add a descriptive name for your form module

13. Click on 'Save & Close' when you're satisfied with your form.

14. Success! Your 'Module saved' message confirms everything worked perfectly.
