1. Click 'Create' and select 'Zap' to start automation.

2. Click 'Trigger'.

3. Search and select 'Slack' as the trigger app.

4. Choose a trigger event based on your requirements.

5. Log in and authorize Zapier to access your Slack account.

6. Click 'Test trigger' and Zapier will pull sample data from Slack to ensure everything is set up correctly.

7. Search and select 'Formstack' as the action app.

8. Choose an action event.

9. Connect your Formstack account by clicking 'Sign in' or 'Change'

10. Select 'Authprize to grant Formstack access to Zapier.

11. Choose your form and provide the details for your action app. Click 'Continue' once you're satisfied.

12. Click 'Test step' and publish your Zap.

13. That's it! You have successfully integrated Slack with Formstack.
