1. Go to your Tally Dashboard and click '+ Create form'.

2. Name your form and customize it however you like it.

3. Click 'Publish' to share your Tally form.

4. Once you have published your form, select the 'Integrations' tab.

5. Click 'Connect'.

6. Click 'Log in' to allow Tally to connect with Slack.

7. Select 'Allow' to grant Tally Forms access to your Slack workspace.

8. You have successfully integrated Slack with Tally.

9. Tally Forms allows Slack to enable you to receive form submissions, updates, and notifications directly in your Slack workspace.
