1. Go to the survey that you want to add a disqualification page and click the ellipsis icon next to its title.

2. From the drop-down menu, select 'Edit survey'.

3. Go to the 'Design Survey' tab in SurveyMonkey and select the screening question you created.

4. Click on the screening question to edit it.

5. Look for the option labeled 'LOGIC' and click on it.

6. If the answer is no, click the option to send the respondent to the 'Disqualification Page.

7. After setting the skip logic to a disqualification page, click 'SAVE'.

8. Click 'Preview survey'.
