Navigate to your desired survey and click 'Send survey'.
Go to the 'Collect Responses' section of your survey. Click the ellipsis icon next to your collector.
Click 'Edit collector'.
After clicking the collector's settings, click 'Show advanced options'.
Click 'Custom Disqualification' and that's the way through where you can edit the disqualification page.
