How to Insert a Document Upload Field into SurveyMonkey Forms

Head over to the top menu bar, then click 'Design Survey'.
After that, go to the editor panel and enter the document upload question into the provided input field.
Click the drop-down menu next to it to change the answer type.
Subsequently, find and select 'File Upload' from the available options.
Configure the File Upload settings based on your preference.
Once done, click 'Save' to apply the changes.
After completing these steps, the Document Upload field will be inserted into your SurveyMonkey form.
