1. Go to your Jira dashboard and in the top navigation bar, click the 'Create' button.

2. In the 'Create Issue' screen, select the appropriate project from the 'Project' dropdown.

3. In the 'Issue Type' dropdown, select 'Bug'.

4. In the 'Summary', provide a clear, concise title for the bug.

5. Fill in the bug details such as custom form, severity, root cause and resolution.

6. Once you’ve filled out all the necessary fields, click the 'Create' button to save the bug.

7. Click 'View issue', the bug will now appear in the backlog or issue list for the selected project.

8. Following this process ensures that bugs are properly documented and assigned, helping your team address and fix issues efficiently.
