Log in to ConvertKit and click '+ Create a new'.
Select 'Form'.
Select a display format.
Click on the add icon to add a custom field.
In the form editor, click on an existing field.
Select a field.
Check the 'Required' box.
ConvertKit automatically saves the new field and click on 'Save' to apply changes.
