1. Go to the top menu bar, then click 'Apps'.

2. Select 'Explore More Apps' from the list of options.

3. After that, search for 'SurveyMonkey' and then select 'SurveyMonkey+ For Jira' from the suggested results.

4. Tap 'Try It Free' at the top-right corner.

5. Subsequently, click 'Start Free Trial' to proceed.

6. In Jira's project dashboard, choose and open any Issue.

7. Following that, scroll down to the Embed section and click 'SurveyMonkey'.

8. Paste the shareable link of your SurveyMonkey form into the provided input field.

9. Once done, tap 'Save' to apply the changes.

10. After completing these steps, your SurveyMonkey online form will be integrated with Jira.
