1. Click on 'Create' and select 'Zaps' to start a new automation.

2. Click the 'Trigger' option.

3. Search for and select 'Google Forms' as the trigger app.

4. Select "New Response' as the trigger event.

5. Choose an account that you like to continue with Zapier.

6. Select 'Allow'.

7. You have successfully granted access to Zapier, click 'Change' if you want to change the account connected to Zapier.

8. Choose the Google Spreadsheet that stores the form responses.

9. Search for and select 'Notion' as the action app.

10. Select 'Create Database Item' as the action event.

11. Select fields from the Google Forms responses to the appropriate Notion database fields.
