Guide

1. Click on the 'Views' option

2. Click on the 'Form' option

3. Click on the 'Create a form view' option

4. Click on the 'Create a new view' option

5. Enter all the details of the questions you’ll need in the form

6. To add additional field click on the 'Add a field to this table'

7. Press the 'Submit' button to create a form
