1. Head over to the left-side panel and then click 'Settings'.

2. Select 'Integrations' from the list of options under Integrations.

3. After that, scroll down to the Embedded Apps section and click 'Create Embedded Apps'.

4. Come up with a unique integration name and enter it afterwards into the provided input field.

5. Next, click the URL text field and paste the shareable link of the SurveyMonkey form you wish to integrate.

6. Configure the integration settings based on your preference.

7. Once done, tap 'Save'.

8. After completing these steps, your SurveyMonkey form will be integrated with Copper.
