1. Open your desired survey.

2. Go to 'Design Survey' tab.

3. On the left-hand side, go to 'Styles' and select 'Themes'. Under 'Themes', click 'Create custom theme'.

4. In the theme editor, look for the 'Background' option.

5. To add background, click '+ Background'.

6. Choose a file from your computer or image gallery to use as the survey’s background.

7. Click 'Done' to add the background.

8. After uploading your background image, click the 'Preview survey' button to ensure the image displays correctly and doesn’t interfere with the readability of your survey.

9. Click 'Continue' and your new background image will now be applied to your survey.
